Ipamorelin binds to and activates the GHSR-1a receptor on pituitary somatotroph cells, triggering a pulsatile release of endogenous growth hormone. This mimics the body's natural GH secretion pattern, avoiding the supraphysiological spikes associated with exogenous GH administration. Ipamorelin is the most selective growth hormone secretagogue available: it stimulates GH release without significantly affecting cortisol, prolactin, or ACTH, making it ideal for protocols where hormonal collateral effects are undesirable. This selectivity is its primary differentiator from other GHRPs.

Ipamorelin is generally well-tolerated with a minimal side effect profile, which is one reason it remains a preferred secretagogue. The most commonly reported effects are mild water retention in the first 1–2 weeks (due to GH's anti-diuretic effect), occasional morning fatigue at high doses, and mild transient insulin-like sensitivity changes. Tingling in the extremities (carpal tunnel-like sensations) can occur and typically resolves with dose reduction. Unlike GHRP-2 and GHRP-6, Ipamorelin does not stimulate appetite, making it preferable for fat-loss oriented stacks. Tolerance to its GH-releasing effect can develop with continuous use; cycling is recommended.
