By inhibiting NNMT, 5-Amino 1MQ prevents the methylation of nicotinamide: preserving it for conversion to NMN and ultimately NAD+. This increases intracellular NAD+ levels, activates SIRT1, and promotes AMPK-mediated mitochondrial biogenesis. In adipose tissue specifically, NNMT inhibition reverses the metabolically inactive 'white adipose phenotype,' reducing adipocyte size and increasing the expression of thermogenic and oxidative metabolism genes. Studies have shown that NNMT inhibition reduces adiposity, improves insulin sensitivity, and extends healthspan, without caloric restriction.

NNMT (nicotinamide N-methyltransferase) consumes SAM (S-adenosylmethionine) to methylate nicotinamide, diverting it from the NAD+ synthesis pathway. 5-Amino 1MQ blocks this enzymatic drain, preserving nicotinamide for NAD+ production and simultaneously preserving SAM for other methylation reactions.
Unlike NAD+ precursors (NMN, NR) that provide substrate directly, 5-Amino 1MQ increases NAD+ by blocking its degradation: a fundamentally different approach that some researchers argue produces more sustained intracellular NAD+ elevation. The combination of both approaches may produce synergistic NAD+ restoration.
NNMT is significantly overexpressed in obese white adipose tissue, where it drives a metabolically suppressed, energy-storage-biased phenotype. NNMT inhibition reverses this, activating oxidative metabolism genes in adipose tissue and promoting a phenotypic shift toward smaller, metabolically active adipocytes.
Studies have reported reductions in fat mass and body weight without changes in food intake, consistent with metabolic activation of adipose tissue. This supports research into a fat-loss mechanism independent of appetite-suppression pathways.
SIRT1 activation from increased NAD+ availability, combined with improved adipose metabolic function, is studied for its relationship with insulin sensitivity. Multiple studies have reported improved glucose tolerance and reduced fasting insulin after 5-Amino 1MQ exposure.
By blocking NNMT's consumption of SAM (the universal methyl donor), 5-Amino 1MQ preserves methyl groups for epigenetic regulation, creatine synthesis, phospholipid metabolism, and other critical SAM-dependent processes: adding a secondary mechanism beyond NAD+ alone.
